Adds a packaged babysit skill and Claude Code slash command for PR merge-readiness.

Changes:

• Adds skills/babysit/SKILL.md with a PR-first workflow: resolve the PR, fetch existing review threads/reviews/comments, run coderabbit review --agent, fix/validate/push, and re-check.
• Adds commands/coderabbit-babysit.md for Claude Code.
• Documents /babysit in README and CHANGELOG.
